Consent form for Research Study
Study Title: Effects of Paleo and Low carbohydrate nutrition on health
The purpose of the study is to understand more about your paleo and low carbohydrate nutrition and how it impacts your health and weight loss.
This survey is designed to help begin to uncover this information. The study researcher, Larry Istrail, from Virginia Commonwealth University is conducting this study.
Why is this study being done?
The purpose of this study is to better understand how paleo or carbohydrate-restricted nutrition impacts your health, weight loss, and overall well being.
How many people will take part in this study?
About up to 10,000 people may take part in the study.
What will happen if I take part in this research study?
If you decide to participate in the study, you will be asked to answer questions about your personal experiences with your diet. The survey should take about 20-30 minutes to complete.
Study location:
The program is delivered online.
Can I stop being in the study?
Yes. You can decide to stop at any time.
What side effects or risks can I expect from being in the study?
We do not anticipate any risks or discomfort to you for participating in this research. While there is a small chance that the confidentiality of the information collected could be compromised, we will take care to prevent this from happening.
Are there benefits to taking part in the study?
There is no direct benefit to you expected from your participation in this study. In the long run we hope that the results of this study
will benefit others and the larger society by providing a better understanding of the impact of various diets.
Will information about me be kept private?
You are participating in this study anonymously, meaning that we do not have information regarding your name or other identifying
information. If you agree to be contacted in the future, we will also have your e-mail address.
